WAIVER OF LIABILITY: This test instrument is designed to be used in conjunction with in-plant or on-site observations of an operator's industrial truck operating practices as part of a comprehensive assessment program. An operator's score on this examination does not insure that safe operating practices are being followed in the workplace. Observations and performance evaluations by supervisors are required in order to properly assess an operator's fitness for safe industrial truck operation. This examination may help to assess the effectiveness of classroom training that has been provided to the operator and may help to identify areas or topics in which an operator may need additional training or re-training. This examination is not a certification device and no particular score constitutes "passing" or "certification" of an operator. The certification or licensing of an operator of industrial trucks is the sole responsibility of the employer. It is the responsibility of the employer to train, supervise, and evaluate operators as to their fitness to operate industrial trucks in the particular environment and under the circumstances that may exist in their workplace. An employer's election to utilize this testing instrument as part of a program for assessing an employee's knowledge of safe industrial truck operation is done with the expressed understanding that no responsibility or liability for the results or actions of any employee accrue to Safetytest.com and the Lift Truck Operator Safety Review, its designers, or its owners.

Enter the general postmaster e-mail address for your company's domain or for the administrator of your mail server, such as; postmaster@company.com, or admin@company.com, etc. In the event that specific individuals' e-mail addresses were to become inactive at any time in the future, we would use the generic address that you provide here, to send important notifications from safetytest, until new individuals' addresses are provided to us.

If you are an individual who is registering as a self-employed individual to use Safetytest, we suggest entering an e-mail address of your spouse or another friend or relative who could forward messages from Safetytest to you in the event that your personal or business e-mail address were to become inactive in the future.
